2021 HKCS Resources

Thank you for your interest in the Healthy Kids Colorado Survey! Here you will find the survey instruments for the 2021 survey cycle. Please note that there are differences between the high school and middle school surveys for the HKCS. In addition, any school district can modify the survey to meet the needs of their community before July 1. If you have questions or concerns about the version of the survey being administered in your district please contact your administrators or email our team at CSPHSurveyTeam@cuanschutz.edu.
