How to Request Local Data

Thank you for your interest in 2023 Healthy Kids Colorado Survey (HKCS) and Smart Source data!

Learn more below about how to access local data, available standard reports, and custom report inquiries.
 

Schools and districts own local data. Please discuss data sharing with your local district first, then send a signed Data Use Agreement (DUA) to CSPHSurveyTeam@CUAnschutz.edu for electronic access to reports.

Standard Reports for the HKCS and Smart Source are the most commonly requested. Brief descriptions are included below, and video tutorials for reports can be accessed via the links.

  • HKCS Frequency Report  [video tutorial] – longest report, includes all indicators, Excel spreadsheet​

  • HKCS Snapshot  [video tutorial] – shortest report, includes select indicators only, local compared with regional (when available) and state data, PDF format​

  • HKCS Trend Report – available at district level only, after participation in 3+ survey years, includes all common indicators & correlating trend​

  • Paired Indicator Report  [video tutorial] – for schools that complete both the HKCS & Smart Source, short report that pairs select indicators by topic area​

  • Smart Source Immediate Report  [video tutorial for all Smart Source reports] – returned within one week of submission, compares school response to best practice​

  • Smart Source Final Report – two reports (one spreadsheet, one PDF), local compared with regional (when available) and state data​

Custom Reports may be requested by contacting CSPHSurveyTeam@CUAnschutz.edu. Examples of custom reports include specific geographic areas such as a county that combines several districts, or specific demographic stratifications or indicators. Note that a separate, signed DUA will be needed for each district included in a custom report.
